    What is your stream name?

    What is your Twitch bot account name? This needs to be a valid Twitch account that you own (meaning you have the Twitch login credentials for it) other than your streamer account

    What is your streaming schedule? [Days/Approximate time]
        -Was originally Fridays through Sundays, starting at 7pm EST.  Now that college classes are over, I am going for at least 5 days a week, starting around 4pm-5pm and streaming for as long as I can

    Is your stream partnered with Twitch?
        - Not yet.  Have an application out to be reviewed though

    What is the average number of users in your channel?
        - approx. 5 - 10

    Do you need to migrate an existing points system over to DeepBot? If so, please specify which bot you are currently using.
        - Yes, from Revlobot

System Edit : Incorrect bot account. Please provide a valid Twitch account that you own and which the bot can use to connect to the Twitch servers with. This cannot be the same account as the stream account.
