Discord/Custom command targeting

Do custom commands when used in discord still only hit those listed in twitch chat?
IF so anyway to change this?

Correct. It is a twitch bot, not a discord bot. The discord integration is meant to run commands from discord to make it a bit easier for the streamer or other users that want/need to run commands.
